Position Summary

Under general administrative direction, coordinate and manage treatment plans for patient with different medical conditions; provides a significant level of patient education related to illness and planned management and treatment. This role involves overseeing the INR (International Normalized Ratio) Clinic, Inclisiran injection clinic, facilitating communication among multidisciplinary teams, coordinating patient care, ordering necessary labs and tests and assisting providers in addressing any clinical insufficiencies. The Navigator serves as a central point of contact for patients, ensuring continuity of care and optimizing patient outcomes.

Essential Duties and Responsibilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned.

  • Serves as a liaison between patients, providers and various healthcare trams to ensure seamless coordination of care
  • Address barriers to care, including scheduling conflicts, insurance concerns and patient education
  • Monitor and manage anticoagulation therapy for patients in the INR clinic
  • Educate patients on anticoagulation therapy, medication adherence, dietary consideration and potential complications.
  • Adjust anticoagulation therapy as per provider protocols and communicate necessary changes to patients
  • Maintain accurate records of INR results and therapy adjustments.
  • Work closely with cardiologist, primary care physicians, and other specialists to ensure comprehensive patient management.
  • Follow up and communicate test results to patient and next steps
  • Provide appropriate clinical support, counseling, and education. Alerts provider to urgent situations that cannot wait until scheduled follow up.
  • Assist cardiology and other departments in identifying and addressing gaps or insufficiencies in patient care
  • Ensure proper documentation of patient encounters, lab results, and treatment plans in the electronic medical record (EMR).
  • Work with multi-disciplinary team to develop a plan of care focusing on reducing readmissions and addresses chronic disease conditions and social/environmental concerns that may lead to repeat hospital stays/visits.
  • 5% of nurse navigator responsibilities will be dedicated to clinical responsibilities
  • Review records needed for patient from EPIC
  • Order lab work and other procedures as needed
  • Assist providers in addressing insufficiencies in care provision by coordinating follow up, orders, and referrals

Minimum Qualifications

Education       

  • Bachelor in Nursing. 
  • Current D.C. Licensure as a registered nurse or evidence of application for licensure in D.C.

Experience      

  • A minimum of 2 years of experience as a registered nurse.
  • Experience in public health or health/patient education preferred. 

Physical Requirements  

  • Walk, stand, and reach above the shoulder frequently in an office setting.
  • Must be able to occasionally lift or carry and frequently push or pull up to 100 lbs. as part of the role.
  • Regularly exposed to healthcare settings that may require personal protective equipment.
  • Requires manual dexterity to operate a computer keyboard, calculator, copier machine, and other equipment.
